While it’s not mandatory to fill out these forms in advance for us to begin the process, submitting them electronically (via fax or email) can help expedite the process and ensure the quickest possible release time for your friend or loved one. In most cases, only one form is needed. Please call us before starting to get specific instructions on which form to use, and which fields are required.
